Service Hour Form Probably the most used form in our school! Students must obtain the signature of a supervising adult in order to get credit for completing service hours. 20 hours are required of all 9th, 10th and 11th graders. Seniors will be participating in group projects, but may complete additional hours for scholarship or Challenge Program goals. Anticipated Absence Form When families are going away during the school year, and students will be missing more than one day of school, then students are required to have this form signed by all teachers. A copy should be submitted to Mr. Graffte, the Attendance Officer, before the student is absent. Failure to do so carries academic penalties. Anticipated Absence Form

Guest Permission Form  When students want to bring a friend from another school to a BC dance, they give this form to their friend who will then have it signed by their own principal. The BC student will then bring the signed form to school and turn it in to the faculty moderator for that dance. If a BC student wants to bring a friend who has graduated from high school already, you must see Mrs. Wolfe for specific permission. No guests over age 21 will be permitted at BC dances under any circumstances. Guest Permission Form

Permission Slip Every trip outside the buildin requires a permission slip. Teachers and moderators always hand these out when they announce a field trip. It seems, however, that this is the most-commonly lost/forgotten form in the history of education. Now, print your own, fill it out, sign it and send it in! Permission Slip

Excuses Any time a student is absent, he/she must bring in a written excuse from a parent or guardian. This form is not necessary--a hand-written note will suffice. This form, though, helps to ensure that all relevant information is included. Excuse Form

Athletic Forms Many forms are required by both the school and the PIAA in order to be eligible for athletic participation. Please complete all forms and turn them in to Coach Craig or the Athletic Office prior to the deadline for each season.

The PIAA Physical Form must be started by the family and the student, and completed by a physician. PIAA Physical Form
